WOUND CARE CALNOC SURVEY
CALNOC STANDS FOR COLLABORATIVE ALLIANCE FOR NURSING OUTCOMES AND HAS BEEN IN THE FOREFRONT OF PROVIDING ACTIONABLE INFORMATION AND RESEARCH ON NURSING SENSITIVE INDICATORS TO ITS MEMBER HOSPITALS FOR THE PAST 18 YEARS. KAISER SANTA CLARA HAS JOINED FORCES WITH CALNOC AND THROUGH OUR WOUND CARE DEPARTMENT WE HAVE DONE QUARTERLY PRESSURE ULCER PREVALENCE AND INCIDENCE RESEARCH. WE COLLECT CRITICAL DATA ON OUR PATIENTS FOUR TIMES A YEAR HOSPITAL WIDE AND THAT DATA IS USED IN THE RESEARCH STUDY. THE PURPOSE OF WHICH IS TO TRACK OUR PROGRESS ON HAPI’S AND MORE IMPORTANT TO LEARN NEW METHODS OF HAPI PREVENTION. I’M PROUD TO HAVE PARTICIPATED IN MANY SURVEYS EVERY YEAR FOR THE PAST SIX YEARS.
TEAMWORK AND COMMUNICATION
TEAMWORK IS AN ESSENTIAL PART OF OUR NURSING PRACTICE IN THE SDU AND ICU. I HAVE HONED MY TEAMWORK AND COMMUNICATION SKILLS OVER THE PAST 27 YEARS WORKING IN ACUTE CARE BOTH AS AN RN AND AN RRT. I HAVE INCREASED MY SKILL AND KNOWLEDGE OF TEAMWORK AND COMMUNICATION WITH CERTIFICATION FROM IHI AND TEAM STEPPS.
NURSING EXCELLENCE
STAFF EDUCAITON AND IN-SERVICE ON NEW CLABSI PROTOCOL
I IDENTIFIED CLABSI AS A PROBLEM WHEN WE STARTED HAVING HIGHER RATES IN OUR DEPARTMENT. AS A STAFF NURSE III, I IDENTIFIED THE NEED FOR STAFF EDUCATION ON KAISER’S NEW CLABSI PROTOCOL. I HELPED WITH THAT EDUCATION AND IDENTIFIED A WAY TO QUICKLY ASSESS WHO HAD A CENTRAL LINE ON OUR FLOOR. I TAUGHT THE STAFF HOW TO DOCUMENT CENTRAL LINE CARE, CHANGE THE DRESSING, CARE OF THE LINE, AND EDUCATION TO BE GIVEN OUT TO THE PATIENT IF THEY WENT HOME WITH A CENTRAL LINE. AS A STAFF NURSE III NURSING EXCELLENCE IS ALWAYS ON MY MIND. FOLLOWING EVIDENCE BASED PROTOCOLS, ACTING AS A COACH, MENTOR, AND INSTRUCTOR IS NURSING EXCELLENCE PERSONIFIED.
